Before You Dive
All participants must review the PADI Medical Statement form and complete the medical history questionnaire before joining any course.
Concealing past medical conditions or illnesses may lead to underwater emergencies that could endanger other participants. Please be honest in your disclosure.
Please call us first to discuss any applicable conditions with our staff.
If a physician's clearance is required, please submit a medical certificate using the official PADI Medical Statement form.

No physician's clearance is required. Please proceed with the registration process.
A physician's clearance is required even if you are in good health. Please visit an internal medicine doctor for an examination.
If false information is provided despite having applicable medical conditions, we cannot be held responsible for any accidents arising from such conditions.
Participation while hungover is not permitted.
In cases of fatigue, sleep deprivation, or poor physical condition, diving may be refused at the instructor's discretion.
Diving may be cancelled or the dive site changed due to weather conditions, wind direction, sea conditions, or operational circumstances.
A parent or guardian's signature is required on necessary documents.
If the parent or guardian is not accompanying the minor, please complete and bring the "Diving Liability and Consent Form for Minors" in advance.
